When you are ready to book an appointment simply call the Practice where you will be asked to provide the following information:
- Your full name
- Date of birth
- Postcode
- Mobile telephone number
- Your GP details.
Once a suitable appointment time has been organised for you, you will receive a text message or email to confirm your booking with us.
What to bring for your first appointment
Anything relevant to your condition, including:
- List of medications
- MRI scans
- X-rays.
If you are covered by medical insurance, please bring details of your insurance company including your registration number and their address and telephone number.
First Appointment
Your first appointment with us will last for up to 45 minutes and upon arrival you will be asked to complete a registration and consent form before you see the Physiotherapist. Once completed you will move into one of our treatment rooms where the Physiotherapist will conduct a full initial assessment of your symptoms before agreeing and commencing your treatment.
We ask that you wear comfortable clothing and if your injury/problem is related to your leg, knee or ankle that you either wear, or bring shorts with you. Please note that it may be necessary for you to be undressed down to your underwear for assessment purposes, so if you have any concerns regarding this you can discuss this with the Physiotherapist on the day.
Cancellation policy
If you are unable to attend a pre-booked appointment, we would be grateful if you could let us know as soon as possible. Please note that appointments cancelled with less than 24 hours’ notice will be charged at the FULL amount.
